The total RVU formula combines physician work, practice expense, malpractice expense, geographic adjustment, and the annual conversion factor into a Medicare payment estimate. The formula is simple only if each assumption is kept separate. A professional RVU calculator should show CPT code, setting, GPCI locality, participant status, and conversion factor year so the final number can be reproduced and audited.

Start with the three RVU components

Every CPT or HCPCS payment estimate starts with RVU components: work RVU, practice expense RVU, and malpractice RVU. Work RVU represents physician work. Practice expense RVU represents resources and overhead assigned to the professional service. Malpractice RVU represents professional liability expense assumptions. Total RVU is built from these components; it is not a single standalone value pulled out of context.

Choose the correct PE RVU setting

The formula requires one practice expense value, not both. For many services, CMS provides non-facility and facility PE RVU values. The selected setting can materially change total RVU while work RVU stays the same. That is why the setting should be chosen before calculating payment and should remain visible in copied results.

Apply GPCI component by component

For locality-adjusted payment, the formula is: work RVU times Work GPCI, plus selected PE RVU times PE GPCI, plus malpractice RVU times MP GPCI. This produces adjusted total RVU. Each GPCI value adjusts only its matching RVU component. Using one blended geography factor can hide why a locality changes payment.

Multiply adjusted total RVU by the conversion factor

After adjusted total RVU is calculated, the annual conversion factor turns RVU into a Medicare payment estimate. In RVUinUSA's 2026 model, non-QPP estimates use $33.4009 and QPP/APM estimates use $33.5675. The conversion factor is the final multiplier; it does not change CPT RVU values, setting, or GPCI locality.

Example formula structure

A complete estimate can be written as: ((Work RVU x Work GPCI) + (selected PE RVU x PE GPCI) + (MP RVU x MP GPCI)) x conversion factor. For national baseline estimates, the GPCI values are 1.000, so the formula is easier to read. For market-specific estimates, locality GPCI values explain why payment differs from the national result.
